Job summary

Nampa of Cascadia, LLC seeks a Social Services/Discharge Planner to advocate for residents, coordinate discharge processes, and support post-discharge needs. You will work with families, physicians, and staff to ensure smooth transitions and comprehensive care planning.

The role requires strong communication and organizational skills, valid driver's license, and preferred MSW with LCSW/LSW licensure. Post-acute/skilled nursing experience is preferred, and all state checks apply before patient

Qualifications

• Demonstrates strong verbal and written communication and strong organizational abilities.

Responsibilities

• Acts as advocate for residents and coordinates discharge planning with families, physicians and staff.
• Maintains clinical documentation on residents referred to social work.
• Provides information to residents and families regarding community agencies and support services.
• Serves as liaison between residents, families and community agencies.
• Assists in development of the total plan of care and case conferences for residents referred to social services.
