Job summary

Saint Peter’s Healthcare System is seeking a Discharge Planning Assistant in New Jersey to support seamless patient transitions from hospital to post-acute settings. The role involves coordinating referrals, authorizations, transportation, and information flow among providers, patients, and families.

Responsibilities include updating staff on Medicaid and insurance details, scheduling follow-ups, and utilizing EMR documentation.

Qualifications

• Bachelor's Degree in Social Work, Social Sciences, or related field.
• Strong interpersonal and communication skills required.
• Ability to organize and manage multiple time sensitive tasks required.
• Ability to simultaneously cover multiple patient care assignment areas reflecting various ages, populations, and diagnoses.
• Hospital experience and direct experience with patients desirable.

Responsibilities

• Initiate referrals for subacute, acute, psychiatric, homecare, and DME to identified providers.
• Print, copy, and fax documentation and files.
• Obtain authorizations when appropriate.
• Coordinate transportation for patients and being the point of contact.
• Communicate with various providers, facilities, and the patients' families to obtain information about patients' previous placements and plan of care upon discharge.
