Job summary

Wellstone Regional Hospital, a 100-bed acute care facility in Jeffersonville, Indiana, is seeking a full-time Discharge Planner for its Social Services Department. The role centers on individualized discharge and safety plans and coordinating follow-up in the community.

The position requires a Bachelor’s Degree and CPI/CPR knowledge, with preference for 1 year of psychiatric or mental health experience. The role emphasizes service excellence and teamwork within a Monday–Friday schedule.

Qualifications

• Completes majority of discharge plans on patients admitted and discharged from WRH Acute Services.
• Documents discharge planning and crisis/relapse prevention planning during discharge.
• Identifies social service and treatment resources within WRH communities.
• Reviews follow-up treatment to ensure smooth transition from facility to community.
• Attends interdisciplinary and social service team meetings as needed.

Responsibilities

• Completes discharge plans per policy for patients admitted and discharged.
• Coordinates follow-up treatment and community referrals.
• Maintains contacts within communities served by WRH.
• Outreach to community agencies to facilitate transitions.
• Performs other duties as assigned.
