Job summary

University of Utah Health Hospital in Salt Lake City is seeking a discharge planning specialist to coordinate care, working under a nurse case manager or social worker. You will liaise with home care agencies, post-acute facilities, and other partners while ensuring compliant, quality care.

The role requires flexibility for rotating shifts, strong communication, and attention to patient outcomes data. This position does not provide direct patient care but supports care coordination across a 24/7

Qualifications

• Completion of a hospital provided medical terminology course within six months of hire.
• Two years of college level education in healthcare or a related field.
• One year of experience in a utilization review or case management environment.
• Basic Life Support Health Care Provider card through American Heart Association may be required within one month of hire.

Responsibilities

• Implements patient discharge plans coordinating with home care agencies and post-acute facilities.
• Collaborates to confirm authorization for services is in place.
• Assists in collecting data related to patient outcomes and auditing data.
• Prepares documents/reports for internal/external groups in care coordination and data collection.
• Documents activities and progress in medical charts and billing/utilization systems.
• Collaborates to address financial advocacy for patient/organization.
• Issues CMS Important Messages to patients/designees.
• Inputs clinical information into computer database.
• May rotate shifts including nights, weekends, and holidays in a 24/7 system.
