Job summary

Johnson Memorial Hospital in Connecticut is seeking a part-time LMSW Social Worker / Discharge Planner to join our Case Management team. The Social Worker is responsible for assessing and providing direct Social Work services and interventions to patients and families in the hospital setting upon admission and throughout discharge.

Minimum Requirements include an MSW from an accredited School of Social Work; CT LMSW preferred, CCM and ACCC certifications preferred.

Qualifications

• MSW degree from an accredited School of Social Work.
• CT State Social Work Licensure LMSW preferred.
• Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred.
• Knowledge of Medicare, Medicaid, and managed care regulations and trends.
• Experience with utilization management, home care, community resources, and rehabilitation beneficial.

Responsibilities
