Job summary

FirstHealth of the Carolinas is seeking an experienced Discharge Planner (RN, MSW, or BSW) to coordinate comprehensive discharge plans in Raeford, NC. This is a non-remote, on-site role within a nationally recognized health system.

The position requires nursing or social work credentials with 3+ years of related experience (RN with 3+ years or ADN with 4+ years). Medical Terminology is required; Case Management certification is preferred. Sign-on bonus and relocation assistance offered.

Qualifications

• Bachelor’s Degree in Nursing or Social Work required or Associate Degree in Nursing with relevant experience.
• Medical Terminology required.
• Case Management certification preferred.
• Position requires clinical and care coordination expertise.

Responsibilities

• Facilitates coordination and development of discharge plans with patients, families, and multidisciplinary teams.
• Assesses discharge disposition and communicates changes to resources to promote timely, safe transitions.
• Develops referrals to Home Health, HME, SNF, and other agencies; coordinates with admissions and care departments.
• Acts as liaison to ensure effective communication across disciplines and ensures CMS-compliant documentation.
