Job summary
FirstHealth of the Carolinas is seeking a Discharge Planner (RN, MSW, or BSW) in Raeford, NC. This role requires coordinating comprehensive discharge plans with patients, families, and a multidisciplinary team, and managing referrals to Home Health and other agencies.
The ideal candidate holds a nursing or social work degree with relevant experience, or an associate nursing degree with extended clinical experience.
Qualifications
- Bachelor’s Degree in Nursing or Social Work with a minimum of 3 years nursing/social work experience in medical/surgical, critical care and/or hospice, home health or public health.
- OR Associate Degree in Nursing with a minimum of 4 years of experience in medical/surgical, critical care and/or hospice, home health or public health.
- Medical Terminology required.
- Case Management certification preferred.
Responsibilities
- Facilitates coordination and development of care plans in collaboration with patient, family and multidisciplinary team.
- Assesses discharge disposition, develops and manages referrals to Home Health, HME, SNF and other agencies.
- Promotes timely, cost-effective discharge plans and coordinates communication across departments.
