Job summary

FirstHealth of the Carolinas in Pinehurst is seeking a Discharge Planner to coordinate comprehensive care plans for patients. The ideal candidate will have a BSN with a minimum of three years of nursing experience in critical care or hospice, or an Associate Degree in Nursing with four years of experience.

This position requires excellent communication skills and the ability to work within a multidisciplinary team to ensure timely, safe discharges. Benefits include a sign-on bonus, great packages, and free fitness memberships.

Qualifications

• Minimum of 3 years nursing experience in medical/surgical, critical care and/or hospice.
• Case Management certification preferred.
• This position does not provide clinical hours toward obtaining a LCSW license.

Responsibilities

• Facilitates coordination of individual plan of care with multidisciplinary team.
• Assesses appropriateness of discharge disposition and communicates changes.
• Promotes quality care environment while maintaining fiscal responsibility.
