Job summary

Daily patient contact and family sessions are part of the caseload. The ideal candidate holds a Bachelor's in a related field and at least two years in behavioral health, discharge planning, or case management.

Qualifications

• Bachelor’s degree in social work, mental health, nursing, or related field.
• Minimum two years of behavioral health experience.
• Experience in healthcare or managed care settings preferred.

Responsibilities

• Coordinate discharge planning with treatment team and patients daily.
• Assist with referrals, family sessions, and staffing.
• Collaborate with outpatient providers to ensure post-discharge care.
• Advocate for patient access to services during treatment meetings.
