The LPN Case Manager will be responsible for completing any clinical assessment for patients requiring post-acute services under State Medicaid programs including, but not limited to, Virginia, West Virginia, and Maryland. The LPN Case Manager will be responsible for completing pre-screens, admissions, and insurance authorizations for Valley Health’s skilled nursing facility and transitional care beds.Extra duties include, but are not limited:Assisting Case Management staff with completion of Level 2 (MIMR) assessment.Interviewing patients, family members, and/or caretakers as needed to complete assessments.Maintaining open communication with care management team regarding approval/denial of clinical assessments.Maintaining proficiency in electronic submission programs and awareness of current policies/trends related to clinical assessments for Medicaid services.Assists with the arrangement and follow-up with community resources.
