The Discharge Planner position involves coordinating and implementing discharge planning for patients at the facility while maintaining effective communication with facility staff and community resources. The job also requires accountability for services provided during the discharge planning process. The main responsibility is to ensure a smooth transition for patients from the facility to their respective communities.

Preferred applicants will have experience with coordinating referrals to community agencies to achieve continuity of care once individuals are released from custody. Preferred applicants would have familiarity with Milwaukee County behavioral health, substance use, and other social services agencies who provide service to our population. Applicants should be highly organized and be able to independently prioritize tasks based on needs and moving deadlines. Preferred schedule for this position is four 10-hour shifts. Days of the week are flexible. Two days per week would be at the Milwaukee County Jail and two days per week would be at the Milwaukee County Community Reintegration Center.
