Job summary

Thomas Jefferson University Hospitals, Inc. is seeking a Discharge Planner to coordinate safe, timely discharge planning and arrange post-acute services for patients on the assigned unit.

You will collaborate with inpatient care managers, social workers, and the care team to schedule follow-up appointments and ensure payer requirements are met. This role emphasizes interdisciplinary collaboration, accurate documentation, and liaison with third-party payers to secure necessary authorizations.

Qualifications

• Bachelor's degree in healthcare-related field required.
• 2 years experience in healthcare field; new graduate <1 year acceptable.
• Knowledge of HIPAA and medical terminology; ability to work with diverse groups.

Responsibilities

• Coordinate discharge by arranging post-acute services (home health, DME, transportation, infusion) with care team.
• Obtain payer authorization for level of care, length of stay, and post-discharge services; communicate determinations to care manager.
• Document all discharge planning activities in the medical record for compliance.
• Review Medicare Notice of Hospital Discharge for inpatient and behavioral health admissions.
• Schedule and confirm follow-up appointments and link to orders and payers requirements.
• Act as liaison between patients, families, providers, payers, and community resources.
• Monitor transfers with care management colleagues to ensure patients are returned to origin hospital when appropriate.
• Communicate, escalate, and prioritize transportation needs with transfer center.
