Job summary

Jefferson Health is seeking a Discharge Planner to coordinate safe, timely, and cost-effective transitions of care for patients on the unit. Qualifications

• Bachelor's degree in a healthcare-related field required.
• Minimum 2 years of healthcare-related experience.
• Knowledge of scheduling rules, insurance, Medicare/Medicaid, HIPAA, and medical terminology.

Responsibilities

• Coordinate all post-acute services and appointments for patients on the unit.
• Obtain payer authorization for level of care, length of stay, and post-discharge services.
• Document discharge planning activities promptly in the medical record for compliance.
• Review Medicare Notice of Hospital Discharge with inpatient and behavioral health admissions.
