Allaire Health Services is seeking a Discharge Planner to join our team in the United States. You will plan and direct discharge processes for residents transitioning from post-acute care to home, coordinating with the care team to address social service needs and care plans.

The role includes maintaining records, completing assessments, and serving as a liaison between the facility and the community to ensure smooth transitions and accurate documentation.

Responsibilities

• Plan and direct the discharge plan for residents transitioning from post acute care to home.
• Participate as a member of the team to address Social Service sections of MDS and care plans.
• Maintain social service records
• Complete assessments of new and current residents.
• Acts as a liaison between facility and the community
• Maintain accurate records and report on clients’ status
