Qualifications

• Experience in discharge planning or post-acute care.
• Ability to complete social service assessments.
• Strong record-keeping and interdepartmental communication.

Responsibilities

• Plan and direct the discharge plan for residents transitioning from post acute care to home.
• Participate as a member of the team to address Social Service sections of MDS and care plans.
• Maintain social service records
• Complete assessments of new and current residents.
• Acts as a liaison between facility and the community
• Maintain accurate records and report on clients’ status
