Job summary

Cedar Creek Hospital in Michigan is seeking a Discharge Planner to support the psychiatric case-management department and psycho-educational programming. You will initiate discharge planning, arrange aftercare, and complete discharge paperwork with patients at time of discharge.

The role may include leading psycho-educational groups and other sessions as assigned, contributing to a smooth transition from hospital to community care for adolescents and adults.

Qualifications

• Strong verbal and written communication skills.
• Previous administrative experience preferred.
• Bachelor's degree preferred.

Position Description
