Job summary

Cedar Creek Hospital in Michigan is seeking a PRN Discharge Planner to support the psychiatric case-management team, coordinating discharge planning and aftercare for adolescents and adults.

You may also conduct psycho-educational groups and assist with discharge paperwork to ensure seamless transitions for patients and their families.

Qualifications

• Previous administrative experience preferred — relevant internship experience considered
• Bachelor's degree preferred

Position Description

The Discharge Planner/Coordinator provides discharge planning support to the psychiatric case-management department and supports psycho-educational group programming. Key responsibilities include, but are not limited to:

• Initiates discharge planning and arranges aftercare appointments/referrals.
• Completes all needed discharge paperwork with patients and supports at time of discharge.
• May conduct psycho-educational groups and other groups as assigned.
