Job summary

Lakeview Health in Jacksonville, FL is seeking a Discharge Planner to coordinate safe transitions from healthcare facilities to post-acute care.

Qualifications

• Bachelor’s degree in social or behavioral sciences or equivalent.
• At least two years of residential or outpatient healthcare experience.
• Strong interdepartmental communication skills and administrative knowledge.
• Knowledge of external facilities and programs related to substance abuse and mental health; marketing knowledge preferred.
• If recovering, must have at least 2 years of abstinence.

Responsibilities

• Coordinate and facilitate safe transitions from healthcare facilities to post-acute care.
• Communicate discharge recommendations to patients and staff promptly.
• Coordinate transfers to outside levels of care per discharge plan.
• Document admission information for non-healthcare patients.
• Register and order prescriptions for recommended patients and relay to regional offices.
• Utilize office equipment and software to complete tasks efficiently.
• Maintain professional demeanor and collaborate with colleagues and clients.

We’re seeking a Discharge Planner to add to our dynamic team!

Discharge Planner

The Discharge Planner plays a critical role in coordinating and facilitating the safe and efficient transition of patients from healthcare facilities to appropriate post-acute care settings. This position ensures that each patient’s discharge plan is comprehensive, addressing medical, social, and emotional needs to promote optimal recovery and prevent readmissions. The Discharge Planner collaborates closely with multidisciplinary teams, including physicians, nurses, social workers, and external care providers, to develop individualized care plans that comply with regulatory requirements and payer guidelines.

Responsibilities

• Must be able to contact a patient’s referral source(s) and relay necessary information to the appropriate staff members in an appropriate amount of time
• Must be able to transfer requested documents to the referral source(s) and facilities relating to discharge recommendation
• Must be able to communicate discharge recommendations and coordinate them with the patient or appropriate staff in an appropriate amount of time
• Must be able to coordinate the transfer of a patient to an outside level of care pertaining to his discharge recommendation
• Must be able to relay information to the appropriate Bradford Regional Offices, relating to the patient’s discharge recommendations in an appropriate amount of time
• Must be able to document admission information for all non-healthcare patients (admission date, referral sources, regional offices, etc)
• Must be able to register and order Vivitrol prescriptions for recommended patients and transfer this information to the appropriate Bradford Regional Offices or outside facilities after discharge
• Must be able to use office equipment and technology (SmartCare, MicroSoft Office, email, fax machines, scanners, copiers, phones, reading medical charts, etc.) pertaining to the above tasks
• Must be able to work in a constant state of alertness as to perform the job safely
• Must be able to work in cooperative manner with co-workers, managers, clients, and prospective clients
• Must maintain a professional demeanor and appearance, must not appear impaired or disoriented, but rather must present coherent and articulate speech, kept appearance, alert, oriented, and responsive to patients’ needs and concerns

Qualifications

• Bachelor’s degree in social or behavioral sciences related fields or equivalent combination of education and work experience
• At least two (2) years of residential or outpatient healthcare experience required
• Must possess the ability of high level interdepartmental communication skills and knowledge of administrative work
• Knowledge of outside facilities and programs related to substance abuse and mental health; knowledge of healthcare marketing preferred
• If recovering, must have at least 2 years of verifiable abstinence
