Job summary

Harris Health System, Inc. in Houston seeks a Pharmacist Transition Specialist to optimize medication management across care transitions, ensure accuracy and safety, and educate patients prior to discharge.

You will work with pharmacy, nursing, and medical staff to fill and deliver prescriptions on time, reconcile discharge meds, and provide bedside education, with supervision of Pharmacy Technician Transition Specialists.

Qualifications

• Bachelor of Pharmacy or Pharm.D. degree from an accredited school.
• Two years of related pharmacy experience or PGY1 residency.
• Bilingual Spanish preferred.

Responsibilities

• Deliver medication management across care transitions and ensure safety at discharge.
• Fulfill prescriptions accurately and coordinate delivery and bedside education.
• Collaborate with pharmacy, nursing and medical staff to ensure timely discharge prescriptions.
