Harris Health is seeking a Supervising Pharmacy Transition Specialist to support safe and seamless medication management for patients preparing to leave Ben Taub Hospital.
In this role, you will actively engage patients, reconcile discharge medications, fill prescriptions, coordinate bedside delivery, and provide medication education before discharge. You will also make follow‑up calls to reinforce education, answer medication‑related questions, and help ensure patients continue appropriate therapy after leaving the hospital.
The Pharmacy Transition Specialist works closely with pharmacists, pharmacy technicians, nurses, physicians, and other clinical partners to ensure prescriptions are accurate, safe, delivered on time, and clearly understood. This role also oversees Pharmacy Technician Transition Specialists and supports inpatient pharmacy operations when needed.
