Qualifications

• Bachelor of Pharmacy; degree from an accredited school of Pharmacy.
• Doctor of Pharmacy degree preferred.
• Three years of progressive clinical practice with PGY1 from an ASHP-accredited residency program (or PGY1+PGY2) required or equivalent.
• Texas Pharmacist License or eligibility; licensure within 90 days of employment.

Responsibilities

• Deliver optimal medication management across all dimensions of care.
• Serve as a resource to the interdisciplinary team and other pharmacists.
• Participate in rounds and clinical initiatives; provide education to technicians and pharmacists.
