Qualifications

• Enrollment in an approved nursing program may be necessary for RN/LPN checklist usage.

Responsibilities

• Facilitate patient discharge process as discharge coordinator/PCT.
• Collaborate with case managers and healthcare team to implement discharge plans.
• Assist licensed nursing personnel in daily tasks related to discharge.
• Support discharge-planning activities and patient throughput improvements.
