Job summary

Columbus Springs Dublin is seeking a part-time Discharge Coordinator to support behavioral health patients. You will coordinate aftercare, maintain community resources, and schedule follow-up appointments with providers and therapists.

Ideal candidates hold a bachelor’s degree in Social Work or Counseling, have strong organizational and communication skills, and can draft discharge summaries while collaborating with physicians, nursing staff, and care teams.

Qualifications

• Bachelor’s degree in Social Work, Counseling, or equivalent.
• Experience with psychiatric and chemical dependency patients preferred.
• Strong organizational, communication, and documentation skills required.
• CPR and de-escalation certification required or obtained within 30 days of hire.

Responsibilities

• Provide aftercare coordination for patients requiring ongoing medical or psychosocial support post-discharge.
• Create and maintain updated community resource listings for aftercare services.
• Schedule aftercare appointments identified by the patient’s provider and therapist.
• Provide patients and families with resources related to future care needs.
• Communicate proactively with clients, families, aftercare providers, and referral sources.
• Draft comprehensive discharge summaries and contracts for each assigned patient.
• Collaborate with the interdisciplinary team including physicians, utilization review, and nursing staff.

How you’ll contribute

A Discharge Coordinatorwho excels in this role:

What we’re looking for

Applicants should have a Bachelor’s degree in Social Work, Counseling, or equivalent. Experience may be substituted for education. Additional requirements include:

• Prior experience with psychiatric and chemical dependency patients preferred

• Strong organizational, communication, and documentation skills required

• CPR and de-escalation certification required or obtained within 30 days of hire
