Job summary

Columbus Springs East, part of Lifepoint Health, is seeking an Assessment Lead for behavioral health to join our team on the 2pm-10pm shift. In this role you will coordinate response to service requests, complete assessments, and help determine the appropriate level of care while supporting patients, families and staff.

The ideal candidate holds a Master’s in social work or counseling with an active OH license.

Qualifications

  • Master's degree in social work, counseling, or equivalent with OH license.
  • RN licensure may substitute for master's degree.
  • Strong clinical assessment skills; psychiatric setting preferred.
  • CPR and de-escalation certification due within 30 days of hire.

Responsibilities

  • Facilitates response to requests for services, including initial response, assessment, and referral; determines level of care and admission into services; ensures appropriate follow-up.
  • Conducts initial evaluation, initiates assessment, documents patient symptoms and needs; provides information to MD for LOC determination.
  • Demonstrates excellent customer service skills for patients, physicians, visitors, and coworkers.
  • Possesses knowledge of Emergency Detention paperwork and EMTALA procedures.

How you'll contribute

An Assessment Lead who excels in this role:

  • Facilitates response to a request for services, including the initial response, the assessment, and referral process, the designation of the appropriate level of care, admission into services, and appropriate follow up activity.
  • Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs and provides information to MD for LOC determination.
  • Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers in a prompt and courteous manner.
  • Possess clear knowledge of Emergency Detention paperwork and processes and EMTALA.

What we're looking for

Applicants should have a Master's degree in social work, counseling, or equivalent and a current clinical or social work license in the state of OH. RN licensure may be used in place of master's degree. Additional requirements include:

  • Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred
  • Must have strong clinical assessment skills
  • With proper credentials, supervisory may be required to complete vitals check on patients
  • CPR and de-escalation certification required within 30 days of hire
