Frederick Health Hospital, Inc. is seeking a Discharge RN to coordinate, track, and escalate barriers to discharge. The role works across inpatient units, case management, pharmacy, and transportation to ensure timely progression of medically ready patients.

The RN supports real-time discharge coordination, uses standardized barrier tracking, and communicates with interdisciplinary teams to improve hospital throughput and patient safety.

Qualificações

• RN licensing in MD or compact state.
• Associates degree in Nursing required; BSN preferred or to be obtained within 5 years.

Responsabilidades

• Identify anticipated and medically ready discharges and monitor discharge status.
• Track discharge barriers using a standardized process (medications, transportation, follow-up, DME, consults, diagnostics).
• Coordinate with providers to complete discharge orders when appropriate.
• Collaborate with case management, social work, pharmacy, units, and transportation to address barriers.
• Facilitate timely communication between units and the Discharge Lounge for eligible transfers.
• Escalate time-sensitive barriers through established pathways and provide real-time problem solving.
