Job summary

Banner Health is seeking a Transitional Care Associate to facilitate safe transitions from acute care to post-acute settings at Banner University Medical Center South Campus in Tucson. The role includes coordinating discharges, collaborating with clinicians, and connecting patients with community resources.

The position requires a social work degree or appropriate licensed healthcare credentials, strong communication, and flexibility to cover a hybrid ED schedule, with weekend rotations.

Qualifications

• A Bachelor's degree in social work or related degree or a Licensed Practical Nurse, or a Licensed Respiratory Therapist is required.
• Must demonstrate effective communication and customer service skills.
• Must be able to work flexible hours and weekends on rotation; BLS is required (not for Insurance Division).

Responsibilities

• Facilitates timely discharge/transfer of clients from hospital care to post-acute settings.
• Collaborates with the healthcare team to implement transition of care and discharge plans.
• Performs data collection for performance improvement projects as assigned.
• Documents all interventions in the patient record, including discharge plan elements.
• Assist and support patients and families in post-acute arrangements; perform follow-up calls as needed.
• Serve as a resource for community services and coordinate care across the continuum.
