Job summary

You will conduct advanced discharge coordination, engage patients and families, prepare regulatory discharge documentation, and coordinate post-acute care with real-time electronic tracking. This role acts as an operational extension of the RN Case Manager under delegated authority.

Provides advanced discharge coordination and clerical support for the Care Management team with an emphasis on forward-facing patient and family engagement, regulatory discharge documentation, post-acute coordination, and real-time electronic tracking. The role supports timely, compliant discharge planning and serves as an operational extension of the RN Case Manager under delegated authority.
