Responsibilities

• Outreach phone calls to engage member.
• Coordinate with the clinical team.
• Assist member in identifying personal interests and connecting to non-clinical resources and activities.
• Able to work independently in the community.
• Maintain professional trusting relationships with all providers and resources.
• Advocate for member needs.
• Documentation of client intakes and interactions under S.T.A.R. policy; use Outlook and Teams.

Salary Range: $21.76 To $26.94 Hourly

Purpose of Position:

Our 90-day outpatient wraparound program is designed to support individuals transitioning from psychiatric and substance abuse hospitalization back into the community. The program aims to help participants live independently, prevent rehospitalization, and improve their recovery journey.

This program bridges the gap between round-the-clock hospitalization and independent living. We focus on the first three months after discharge, which is typically the highest-risk period for psychiatric relapse.

Discharge Care Coordination (DCC) support is provided within the community, helping the individual to build lasting connections and providing a variety of meaningful community resources. Coordinating with the client, the client’s clinical team, and family, when possible, to accomplish the client’s best outcomes.

Essential Functions:

• Transitional housing & hospital visits, engage client while inpatient, and complete home visits.
• Participate in the discharge planning, staffing with the inpatient team, outpatient team, and client.
• Arrange and/or provide transportation to community resources and appointments.
• Outreach phone calls to member for engagement.
• Coordinate with the clinical team.
• Assist member in determining personal interests and finding community resources and non-clinical supportive activities that fulfill their needs.
• Able to work independently in the community.
• Maintain professional trusting relationships with all providers and resources.
• Assist member in advocating for their needs.
• Documentation, completing client intakes, maintain and document accurate client interactions and services provided under S.T.A.R. policy.
• Utilize outlook for emails, calendar, and Microsoft team’s communication as directed.
