Job summary

Qualifications

• High School Diploma required.
• Associate Degree preferred.
• 1 year prior healthcare setting experience required.
• Discharge planning, billing utilization review, financial counseling or registration training preferred.
• Develop and maintain good working relationships.
• Work in a high-volume environment while maintaining quality.
• Strong analytical and problem solving skills required.
• Excellent written and verbal English communication skills.
• Ability to manage workload and priorities with minimal supervision.
• Ability to handle moderately to highly complex problems in standard or emergent situations.
• Secretarial experience and strong customer service skills.
• Microsoft Word with typing speed 30 WPM required.

Responsibilities

• Coordinate and manage a wide range of complex, high-volume patient referrals post-discharge.
• Interact with care managers, patients, payers and community care providers.
• Data entry and analysis related to insurance coverage and resources.
• Coordinate post-discharge services including home health, DME, nursing homes, transfers.
• Conduct post-discharge follow-up calls to ensure services are rendered and resolve issues.
• Elicit patient/family feedback and report dissatisfactions for service recovery.
