Job summary

Qualifications

• RN: bachelor’s degree in case management or health care and current RN license.
• Social Worker: MSW and LMSW/LCSW or MSW with licensure within 6 months.
• Basic Life Support (BLS) certification required.
• 2 years clinical experience with strong communication and time management skills.

Responsibilities

• Manages individual patients across the health care continuum to achieve optimal outcomes.
• Leads discharge planning with interdisciplinary team and communicates plan across care continuum.
• Maintains knowledge of Medicare/Medicaid benefits to assist discharge planning.
• Educates health care team on case management concepts and integrated care.
• May supervise other staff.
