Job summary

Pinnacle Treatment Centers, Inc. in Martinsville, IN is seeking a Discharge Planner to lead discharge planning, coordinate with the treatment team, and ensure smooth transitions to aftercare.

You will document plans in the EHR, build relationships with referral sources, and help patients secure stable housing and services. A Bachelor's degree and 3–5 years of discharge planning experience are required.

Qualifications

• Bachelor’s Degree from an accredited college or university; applicable experience may be substituted for a degree.
• 3-5 years clinical case management/discharge planning experience

Responsibilities

• Initiate discharge planning and coordinate with treatment team.
• Identify appropriate aftercare and schedule referrals.
• Maintain relationships with referral sources and community resources.
• Document discharge plans in EHR within two working days.
• Ensure discharge plan includes housing stability and transitions.
• Provide exceptional customer service to internal and external customers.
• Participate in team meetings and complete required training.
• Perform other duties as assigned.
