Job summary

PIH Health Hospital - Whittier seeks a Discharge Coordinator to support the Care Management team with advanced discharge coordination, clerical duties, and post-acute coordination. You will operate under delegated authority of the RN Case Manager to promote timely, compliant discharge planning.

The role requires strong communication, basic computer skills, and ability to navigate multiple electronic systems to track discharge activity and ensure smooth transitions for patients and families.
