Job summary

Qualifications

• Two years RN experience in acute care or case management.
• Completion of an accredited RN program allowing licensure as RN.
• Registered Nurse License (California).
• Basic Life Support (BLS) certification.
• Demonstrated utilization review/management and discharge planning experience.
• Strong communication, critical thinking and problem-solving skills.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Collaborates with physicians and multidisciplinary team to create and document plans of care.
• Recommends alternative levels of care and ensures compliance with regulations.
• Assesses high risk patients for post-hospital care planning.
• Develops and coordinates discharge plans and communicates with physicians, patient, family and community agencies.
• Reviews, monitors and coordinates hospital stay to ensure timely and efficient services.
• Coordinates transfer of patients to appropriate facilities and documents steps taken.
• Educates patients about discharge and coping with illness.
• Participates in utilization review activities and quality management.
