Job summary

Brown University Health reports to the manager of Case Management and, under the guidance of case management, provides assistance to ensure implementation of discharge arrangements for all patients. Responsibilities include acting as a liaison between patient/hospital and outside agencies regarding discharge arrangements and arranging post hospital MD visits.

Qualifications

• Equivalent to a high school diploma.
• One year's experience in a medical setting.
• Computer skills required.
• Ability to work and communicate with a variety of individuals.

Responsibilities

• Functions as a liaison between patient/hospital and outside agencies regarding discharge arrangements.
• Primarily but not limited to arranging post hospital encounter MD visits.
• May participate in departmental studies and projects to include planning and coordinating activities
• Complies with hospital and departmental policies and procedures, including protecting the confidentiality of patient information.
• Performs other duties as assigned.
