Eleos-Wellness is seeking a Discharge Planner to coordinate safe, timely discharge planning from admission through discharge for adults and children served in the CSU, ensuring compliance with the Florida Baker Act, AHCA requirements, and CARF standards.

You will collaborate with the interdisciplinary treatment team, families, courts, and community providers to arrange aftercare, transportation, housing, school coordination for minors, and timely follow-up appointments, with clear documentation

Formación

• Bachelor’s degree in social work or related field; relevant mental health experience preferred.
• Knowledge of Baker Act procedures and AHCA requirements.
• Experience coordinating discharge planning and aftercare services.

Responsabilidades

• Initiates discharge planning at admission and updates the plan throughout stay.
• Coordinates aftercare including transportation, housing, and school coordination for minors.
• Maintains timely, accurate documentation and communicates with the treatment team.
• Monitors Baker Act time clocks and deadlines; alerts team of upcoming dates.
• Prepares discharge summaries, notices, and related court/agency packets.
