Health & Human Services Commission (HHSC) in Big Spring, TX invites you to join as a Social Worker III. You will assess patient needs, coordinate discharge planning, and liaise with families, courts, and providers to ensure safe transitions and ongoing care.

You will lead complex cases under supervision, provide trainings, and participate in interdisciplinary teams. Requires Texas Social Work License and a patient‑centered approach to supporting Texans.

Qualifications

• Six months of experience working directly with people with mental, social, or substance use disorders.
• Knowledge of social work standards and discharge planning processes.
• Ability to coordinate transfers, furloughs, and community placements.

Responsabilités

• Gathers information to assess support systems and service needs.
• Assists with discharge planning, transitions, and post-discharge placement.
• Designs and conducts trainings to promote patient rights and decision‑making.
• Coordinates with patient, legal representatives, and agencies to assure ongoing care.
