Texas Health and Human Services Commission (HHSC) seeks a Social Worker III at Kerrville State Hospital to perform highly complex social services, coordinate care, and support discharge planning for patients with complex needs.

Formación

• Licensed LMSW or LCSW required.

Responsabilidades

• Interviews and conversations to document social history and assess support systems and needs.
• Coordinate and oversee transfers, furloughs, and community placement discharges.
• Designs and conducts individual and group trainings to promote rights, decision-making, and self-advocacy.
• Oversee work of Social Worker I/II and students, including training and evaluations.
