Qualifications

Responsibilities

• Discharge planning and resource information for patients
• Assess psychosocial risk factors and coordinate with care team
• Assist with community-based services referrals for patients
• Collaborate with RN Case Manager on social determinants of health
• Document in Electronic Health Record and track progress notes
• Participate in interdisciplinary rounds to confirm discharge plans
