Job summary

Wellstar Health System is seeking a Care Coordination Social Worker (CC SW) in Georgia to assess psychosocial needs and coordinate patient discharge planning across the continuum. You will engage patients and families, coordinate with physicians and RN Case Managers, and ensure safe transitions to the next level of care while considering social determinants of health.

Qualifications

• Initial psychosocial/functional assessments completed and documented in medical records.
• Assist with understanding insurance and coverage requirements for all payers.
• Collaborate with patient, family, physician and care team to support progression and discharge plans.
• Advocate for cultural or religious beliefs in care and continuity of services.

Responsibilities

• Discharge planning implementation with timely resources.
• Identify barriers to timely disposition and document them.
• Participate in Interdisciplinary Rounds for discharge recommendations.
• Provide post-acute referrals and financial needs assessment when needed.
• Support care team with social determinants of health discussions.
