Responsabilidades

• Implements discharge planning and provides timely resource information for complex patients.
• Identify barriers to timely disposition and document them.
• Coordinate with payers to meet insurance requirements for follow-up care.
• Collaborate with patient, family and care team to support progression and discharge plans.
• Participate in interdisciplinary rounds to confirm discharge date and optimal care level.
• Mentor new staff and precept students as needed.
• Support development of protocols and performance improvement initiatives.

Formação académica

Bachelor's or MSW preferred

GA LMSW / LCSW licensure

BLS certifications (ARC-BLS, Basic Life Support, Instructor)
