Job summary

Willow Creek Behavioral Health seeks a Case Manager to provide case management across units, focusing on discharge planning and aftercare coordination. You will maintain patient records, collaborate with families and providers, and ensure services align with the individualized service plan (ISP).

The role requires collaboration with the clinical team to coordinate care, access community resources, and manage referrals and documentation within required timeframes.

Qualifications

• Bachelor's degree in a behavioral health area, marketing, business administration or related field.
• 1+ year related experience in Behavioral Health preferred.
• Previous experience in case management preferred.

Responsibilities

• Participates in development of the service plan.
• Implements activities and services per the ISP within the case manager's scope.
• Coordinates aftercare and discharge planning with the treatment team and family.
• Maintains comprehensive clinical records and coordinates releases with providers.
• Connects patients to psychiatric, therapy, housing, employment or other placement options.
• Engages with the care team to ensure ISP services are implemented.
• Ensures referrals and documentation are completed timely with providers.
• Makes arrangements for community resources to support discharge.
