Job summary

Willow Creek Behavioral Health seeks an Outpatient Case Manager to support clinical services through case management, discharge planning, and aftercare coordination. Responsibilities include coordinating referrals, maintaining records, and collaborating with families and providers to ensure continuity of care.

The role requires a Bachelor’s degree in behavioral health or related field, and 1+ year of related experience; CPR and de-escalation training are required or provided on hire.

Qualifications

• Bachelor’s Degree in Behavioral Health or related field preferred.
• 1+ years related experience in Behavioral Health preferred.
• CPR certification and de-escalation training required (training provided).
• First aid may be required based on state or facility requirements.

Responsibilities

• Develop and implement the service plan.
• Coordinate discharge planning and aftercare.
• Maintain comprehensive clinical records and documentation.
• Coordinate referrals and connections for mental illness and/or substance use services.
• Collaborate with the clinical team to ensure ISP goals are met.
• Coordinate AHCCCS/SSI/SSD and other entitlements when criteria are met.
• Initiate discharge planning at admission and engage family and providers in planning.
• Schedule aftercare appointments and arrange community resources.
