Placements24 is seeking a proactive Disability Support Officer to join our client’s team in George. This role focuses on providing individualized support to people with disabilities, promoting independence and quality of life through person-centered care and collaboration with families.

You will develop support plans, assist with daily living activities, advocate for needs, and work with allied health professionals to connect clients with community programs, education, and employment

Qualifications

• Relevant qualification in disability services, social work, or a related field.
• Experience supporting individuals with diverse disabilities.
• Strong understanding of disability legislation and person-centered approaches.
• Excellent problem-solving skills and ability to manage challenging situations calmly.
• Good organizational and time-management abilities.
• Commitment to promoting independence, dignity, and inclusion.

Responsibilities

• Develop and implement individualized support plans based on client assessments and goals.
• Assist clients with personal care, mobility, medication management, and daily living activities.
• Facilitate engagement in community programs, education, and employment opportunities.
• Provide emotional support and empower clients to develop coping strategies and self-advocacy skills.
• Maintain detailed records of client progress, incidents, and support provided.
• Collaborate effectively with allied health professionals, support networks, and external agencies.
