Job summary

Indian Creek Foundation seeks a Program Specialist to coordinate services for adults with developmental disabilities in residential settings or home-based services across Montgomery and Bucks Counties. The role requires a bachelor’s degree in a social services field and at least two years of relevant experience.

Qualifications

• Bachelor’s degree; social services discipline preferred.
• Minimum of 2 years’ experience with developmental disabilities.
• Strong written and verbal communication; English literacy essential.

Responsibilities

• Coordinate services for individuals per their ISP and age-appropriate goals.
• Document outcomes and communicate needs to the team and stakeholders.
• Train Residential Instructors on outcomes and health/safety needs.
• Complete Annual Functional Assessments and ISP revisions.
• Monitor regulatory documentation and reporting to the Supports Coordinator.
• Attend ISP/IPP meetings and participate in updates and revisions.
