Job summary

Keystone Human Services seeks a Program Coordinator to coach and support individuals with disabilities in achieving personal goals and community participation. You will model and accompany activities, assist with meeting new people, and help develop friendships and choices while supporting families and staff.

Responsibilities include scheduling, documentation review, and coordination of assessments, while maintaining safety and compliance across programs.

Qualifications

• Requires high school diploma or GED with three months of related work experience.
• Associate or bachelor’s degree with related work experience preferred.
• Ability to communicate both orally and in writing; read and interpret written information.

Responsibilities

• Assists individuals served in utilizing public resources and increasing exposure to life experiences.
• Provides training and support to increase personal competencies and social integration.
• Mentors new employees and assists with documentation, orientation, and scheduling.
• Monitors and validates documentation to ensure outcomes are met; maintains compliance.
