Hammer Case Management is seeking a Case Manager to provide service coordination for individuals with disabilities and varied needs. You will work under the Lead Case Manager, conducting in-person visits and developing person-centered support plans.

The role requires a bachelor’s degree in human services, driving to client homes, and strong written/verbal communication. Flexibility for evenings/weekends is essential; benefits include health/dental/vision insurance and 401(k).

Formación

• Bachelor’s Degree in human services or related field required by DHS.
• Experience in disability services preferred.
• Strong written and verbal communication with clients and professionals.
• Ability to travel to client homes using reliable vehicle.

Responsabilidades

• Take person-centered approach to identify needs and preferences of individuals on caseload.
• Find resources to meet individual needs and complete referrals for services.
• Maintain regular contact with providers to ensure quality service delivery.
• Manage payment authorization for service providers.
• Document case notes in electronic record system with detailed narratives and timestamps.
