Job summary

The State of Nebraska is seeking a Disability and Aging Service Coordinator in Beatrice, NE. You will act as a case manager for individuals eligible for a Waiver and coordinate services under HCBS programs, ensuring health, welfare, and safety.

Responsibilities include assessments, eligibility determinations, service plan development, and referrals. A Bachelor's degree and related field experience are required; a valid driver’s license is needed.

Qualifications

• Minimum qualifications include a Bachelor's degree and related field experience in education, psychology, social work, sociology, human services, or a related field.
• Experience with development disabilities services preferred; strong assessment and planning skills.
• Must possess a valid driver’s license and have access to a vehicle.

Responsibilities

• Coordinate and oversee the delivery of services.
• Conduct assessments and eligibility determinations.
• Develop plans of services and supports.
• Provide appropriate referrals.
