Job summary

The State of Nebraska DHHS is seeking a Disability and Aging Service Coordinator in Beatrice to manage a waiver-based caseload and coordinate services for eligible individuals. The role emphasizes assessment, planning, referral, and ensuring safety and well-being for people served.

Applicants should hold a bachelor’s degree with related field experience; a driver’s license is required and a state vehicle may be provided depending on location.

Qualifications

• Minimum Qualifications: Bachelor’s degree and professional experience in education, psychology, social work, sociology, human services, or a related field and experience in services or programs for persons with intellectual or other developmental disabilities.
• Preferred Qualifications: Experience with person centered plans, assessments, habilitation, program implementation, team meeting facilitation; experience with Word, Excel, Outlook, Nfocus and Therap; experience with services for people with special healthcare needs.
• Other: Valid driver’s license and access to a reliable vehicle; state vehicle may be provided depending on office location.

• Coordinate and oversee the delivery of services.
• Conduct assessments and eligibility determinations.
• Develop plans of services and supports.
• Provide appropriate referrals.
• Monitor provision of services.
• Ensure health, welfare, and safety of those we support.
