Job summary

State of Nebraska is seeking a Disability and Aging Service Coordinator in Grand Island to manage services for eligible individuals. You will coordinate providers, assess needs, and develop plans while ensuring safety and welfare. Strong organizational and communication skills are essential.

This full-time position requires a bachelor’s degree in a related field and experience in developmental disabilities or aging services. A valid driver’s license and reliable vehicle are preferred.

Qualifications

• Bachelor’s degree in education, psychology, social work, sociology, human services, or related field; experience may substitute on a year-for-year basis.
• Experience with person-centered planning, assessments, habilitation, and program implementation preferred.
• Proficiency in Word, Excel, Outlook; familiarity with Nfocus and Therap preferred.

Responsibilities

• Coordinate and oversee the delivery of services to eligible individuals.
• Conduct assessments and eligibility determinations.
• Develop plans of services and supports.
• Provide appropriate referrals and monitor service provision.
• Ensure health, welfare, and safety of those supported.
