Job summary

The State of Nebraska in Scottsbluff is seeking a Disability and Aging Service Coordinator to manage waiver services and coordinate care for eligible individuals under HCBS.

You will conduct assessments and eligibility determinations, develop service plans, provide referrals, and monitor service delivery to ensure health, welfare, and safety. A bachelor’s degree in a related field is required with relevant experience preferred, and a valid driver's license is needed.

Qualifications

• Bachelor’s degree in education, psychology, social work, sociology or related field; substitution allowed year-for-year.
• Experience in services for persons with intellectual or developmental disabilities preferred.
• Candidates graduating within 90 days considered.

Responsibilities
